What to Expect on Your First Visit

We look forward to meeting you! To prepare for your initial visit, we want to share some information with you to ensure you have the best possible experience!

Step 1: Paperwork – Fill this out before to save time

In order to utilize your time well, we’d love for you to fill out our new patient form(s) before coming in for your first appointment. Along with these forms, please bring a photo ID and any applicable insurance. Accepted Insurance Providers include the following: Blue Cross Blue Shield and Aetna.

Step 2: Consultation and Exam

Our doctors will perform a consultation and exam in order to locate the source of any pain you might be experiencing. This helps us determine the best course of action for your treatment. Throughout your treatment plan, we will perform follow-up exams so we can map your progress and improvements.

Step 3: X-Rays & Tests

We will take x-rays for diagnostic purposes. However, if you’ve had x-rays within the past 6 months, we might be able to use your previous images if they cover the areas needed for proper diagnosis. If you have any questions or would like help transferring your x-rays to our office, give us a call!

Step 4: Relief Treatment & Follow Up

If our doctors believe you should be treated on your initial visit, they will go through their recommended treatment plan for you, and decide with you what is right.
